Getting you started smoothly is important to us. When you make your first appointment, we will gather information during our call with you.  We ask about your condition and needs, how you heard about us, who your physician is, if you have insurance coverage, when you are able to come in, and what forms you will need to complete. You can complete and print forms before your first visit or arrive 15-30 minutes before your first appointment and complete them. 

 

Your first visit will take about 90 minutes. During this time we will evaluate and treat your condition. We will also schedule future follow-up appointments as needed based on the treatment plan we put together.
